Experiencing Homelessness?
 

If your family is experiencing homelessness or otherwise without consistent shelter/housing, please enter your nighttime address and a mailing address where you can receive mail. Feel free to fill out a Housing Form that is provided at the end of this enrollment.
 

Enrolled in the Address Confidentiality Program?
 

If your family is enrolled in the Address Confidentiality Program, please leave N/A in the Physical Address section. Please offer a mailing address where you can receive mail. Feel free to upload your Address Confidentiality Program Card at the end of this enrollment.

Is your family/student enrolled in the WA State Confidential Address Program?

If yes, please plan to submit a copy of your CAP card.

Do you have any safety concerns as it pertains to reporting/verifying your address with this City program?

If yes, City staff will contact you directly to verify your program eligibility.
