The questions within this tool are designed to identify households who are most likely to become homeless in the imminent future (within 30 days) without homelessness prevention program assistance. This tool is designed to be completed by program staff or administrators. Clients should not complete this form on their own.
How to apply

Verify if the applying household is eligible for Homelessness Prevention (HP) assistance.
Prior to program enrollment, HP staff will document evidence of eligibility with a copy of the eviction notice, ‘pay or vacate’ notice, rental ledger, or through written or oral communication with landlord or a friend/family member where the presenting household is currently.

Determine priority based on vulnerabilities or housing barriers.
Staff will ask a series of questions of the household to determine the presence of current or past conditions, characteristics, and attributes that are most closely correlated with the incidence of homelessness.

To administer the tool, staff will go through the form and check each option for which the condition, characteristic, or attribute is present or true for the household. Each checked box has a point value associated with it.
- After completing all questions, add up the value of all checked boxes and assign a total score to the household.
- Fill out the rest of the form including the date it was completed and if the person was enrolled in the program or not and why.
